Expression and correlation of hormonal receptors (estrogen and progesterone receptors) and her2neu oncoprotein in invasive breast carcinoma

Abstract

Objectives: To study the immunohistochemical markers in invasive carcinoma of breast and to assess the relationship of hormonal receptor status with Her2Neu oncoprotein overexpression.

Materials and Methods: This was a comparative cross sectional study conducted at Chughtais Lahore lab from Jan 2011 to Oct 2011. Eighty two biopsy proven cases of breast carcinoma were reviewed. Immunohistochemistry was used to evaluate the expression of estrogen receptors (ER) and progesterone receptors (PR) and HER2/neu oncoprotein. The relationship between hormone receptors and HER2/neu protein over expression was assessed using the chi square test. The results were considered statistically significant if the P value was <0.05. All analysis were performed with SPSS version 13.0.

Results: This study comprised 82 cases of invasive breast carcinoma. Out of these 82 cases, 2 were males and 80 were females. Majority of the cases were between 25-45 years age group. Estrogen receptor was expressed in 43 cases (52.4%) while progesterone receptor was expressed in 29 cases (35.3%). Thirty five cases were HER2/neu positive (42.6%).

Considering the association between ER, PR and HER2/neu, ER was expressed in 43 cases, out of which 36 were HER2/neu negative and 7 were HER2/neu positive. Similarly, out of 29 PR positive cases, only 5 were HER2/neu positive and 24 were HER2/neu negative. The correlation between ER and HER2/neu (P value: 0.0001) and PR and HER2/neu (P value: 0.04) was statistically significant.

Out of the 27 cases which were positive for ER and PR, only 3 cases were positive for HER2/neu overexpression. Conversely, of the 37 cases which were negative for ER and PR, twenty eight cases were positive for HER2/neu overexpression. This inverse relationship between hormonal receptor status and Her2/neu overexpression was statistically significant. (P value< 0.05).

Conclusion: ER was the most commonly expressed immunohistochemical marker. ER and PR expression was inversely related to Her2/neu overexpression.
