DIAGNOSTIC ACCURACY OF AUTOMATED CAPILLARY ELECTROPHORESIS
Abstract
Objective: To determine the diagnostic accuracy of automated capillary electrophoresis considering immunofixation as reference standard in adult population
Study design: Diagnostic accuracy study (cross sectional).
Place & Duration of Study: Department of Chemical Pathology & Endocrinology, Armed Forces Institute of Pathology, January 2014 to June 2015.
Materials and Methods: A total of 94 adults of both genders, aged 18-80 years were included in this study. Known patients of multiple myeloma on follow up were excluded. Immunofixation was carried out by manual method while capillary electrophoresis was carried out on automated Sebia minicap. Sensitivity, specificity, positive and negative predictive values and likelihood ratios were calculated.
Results: Sensitivity of automated capillary electrophoresis was found to be 97% and specificity 96% for the detection of paraproteins. Positive predictive value (PPV) was 98% and negative predictive value (NPV) was 93%. Positive likelihood ratio was 1.02 and negative likelihood ratio -1.
Conclusion: Diagnostic accuracy of capillary electrophoresis was found quite comparable to immunofixation for the diagnosis of various disorders of immunoglobulins e.g. multiple myeloma.
